Update 8.6.0
27.10.2026
The All Plagued Day has arrived again!

The Scarlet Angels scouts have discovered an Abandoned Leprosarium. It was once a place where the sick were treated, but now it is being used for experiments with the Gray Decay.

Behind it all is Dr. Darasche-Koh, a physician and alchemist who studies the disease's unusual properties.
In Update 8.6.0, Exiles will explore the Leprosarium, descend into the hidden laboratory beneath it, and face the consequences of the doctor's research.

The formulas found within will allow for crafting new decay weapons in the Elemental Forge.
Update 8.6.0 Review | Grim Soul: Dark Survival RPG
Abandoned Leprosarium

The Leprosarium is a sprawling two-level location, where the ruins of an old hospital are strewn with alchemical equipment.

On the surface, the crumbling wards and other remnants of the old hospital still stand, though Darasche-Koh has repurposed much of the complex for his experiments. Here, you'll find operating rooms, alchemist's tables, reagent stores, and other signs of his grisly work.

The darkest secrets, however, lie underground — in a laboratory filled with vats of infected subjects, elaborate machinery, and the many products of Darasche-Koh's experiments.

You will have to explore both parts of the complex.

During the All Plagued Day event, the Leprosarium will be available in its expanded form for three weeks. After that, it will enter the regular combat-event rotation as a one-week dungeon, without the surface labyrinth and its extra rewards.

Surface Area

On the surface, you'll find numerous rooms with reward chests.

Some doors can be opened with a new item, Active Solution. Others will require explosives.

This is also where you'll find the entrance to the underground laboratory.
Underground Laboratory

The dungeon has three difficulty levels.

On Normal difficulty, you can leave the dungeon during a run. If you die, you can return to collect your belongings. On Heroic and Legendary difficulties, however, you cannot leave until the challenge is complete.

The layout is also different from what you're used to. Almost every room is unlocked from the start, so you can explore them in any order.

Only the final boss's room remains inaccessible. To enter, you first have to deal with Igor the Assistant.

New Enemies
The laboratory's inhabitants are highly resistant to decay damage. Fire, on the other hand, is particularly effective against them, while ice is somewhat less so.

Failed Experiment

Small and swift.
The Failed Experiment is not very dangerous on its own, but Igor can create more of them by activating the laboratory's mechanisms.

Successful Experiment

A more formidable foe whose equipment depends on the selected difficulty. On Normal difficulty, it may fight bare-handed; on Legendary, it wields powerful decay weapons.

The Successful Experiment can also restore its health during combat.

Abomination

A large creature with extremely rapid health regeneration.

Effects that reduce the target's healing, along with powerful fire weapons, will help bring it down.

Gobber

The Gobber attacks the Exile from a distance and tries to keep them at bay.

Rather than moving through the laboratory in a regular way, this enemy shifts between predetermined positions.

Igor the Assistant

Igor is Dr. Darasche-Koh's assistant — and the key to the final boss's room.

Instead of engaging the Exile directly, Igor runs around the laboratory and activates various levers to make the Exile's life difficult.

The levers can release Failed or Successful Experiments, temporarily reduce the Exile's movement or attack speed, deal decay damage, or increase enemies' speed.

To open the door to Darasche-Koh, you'll have to catch and defeat Igor.

Dr. Darasche-Koh

Deep within the laboratory, its master awaits: Dr. Darasche-Koh.

A physician and alchemist, Darasche-Koh studied the properties of the Gray Decay. In the final battle, he puts the fruits of his research to use on himself.

The fight revolves around doctor's two forms.

In human form, Darasche-Koh keeps his distance, hurling alchemical grenades at the Exile. The blasts leave behind lingering clouds that deal decay damage and gradually reduce equipment durability.

At a certain point, the doctor drinks a potion and transforms into a monster. In this form, he switches to melee combat and deals significantly more damage.

The clouds of decay left behind during the first phase do not dissipate, however. You'll have to evade Darasche-Koh's attacks while keeping an eye on hazardous areas of the battlefield.

After a while, the doctor returns to his human form, and the cycle begins anew.

Ronald the Red

The Bone Powder gathered in the laboratory will come in handy back on the surface.

Ronald the Red of the Guardians of Purity awaits you at the entrance to the Leprosarium. He will exchange your Bone Powder for Active Solution.

Active Solution lets you enter infected rooms and reach additional rewards.

New Elemental Forge Formulas

New formulas for decay weapons have been added to the Elemental Forge.

These formulas can be found in the Abandoned Leprosarium and work the same way as the fire weapon formulas you're already familiar with.

Four are for existing weapons:
  • Ritual Dagger
  • Cleaver of a Hundred Diseases
  • Bow of Decay
  • Insatiable Mace
The Ritual Dagger formula is available immediately to all Exiles with an Elemental Forge.

Four more formulas let you craft entirely new weapons:
  • Chimera Bite
  • Poleaxe of the Plague's Chosen
  • Epidemic
  • Scavenger's Knives

New Decay Weapons
All four new weapons combine physical and decay damage.

Chimera Bite

A one-handed morning star.

It has a chance to cause bleeding and deals increased decay damage to poisoned or bleeding targets.

Poleaxe of the Plague's Chosen

A legendary two-handed poleaxe.

Its bonus decay damage begins to increase once the Exile's current health exceeds a certain threshold. The more health the Exile has above that threshold, the greater the bonus.

This makes the poleaxe even more effective when combined with equipment and effects that increase the Exile's maximum health.

However, the poleaxe does not benefit from the increased damage of attacks while sneaking.

Epidemic

A glaive that is particularly effective against groups of enemies.

The more enemies surround the Exile, the more decay damage it deals.

Scavenger's Knives

A set of throwing knives.

After killing an enemy, the next attack with the same weapon deals double damage. Combined with sneaking, this allows for an especially powerful strike against a new target.

New Direwolf Colorings
Ghostly White Direwolf

Some Exiles may already have spotted an unusual ghostly white direwolf among their neighbors' pets.

In Update 8.6.0, this coloring joins the pool of possible All Plagued Day direwolf colorings and can appear on epic pets as well.

Sepulchral Direwolf

Meet the brand new Sepulchral coloring.

Dark fur, glowing green veins, metal accents, and electrical arcs make it one of the most unique direwolves in the game.

The Sepulchral Direwolf is available through a limited-time offer.

All Plagued Day

This update also brings back ghostly pets, themed decorations, and other familiar All Plagued Day activities.

Some existing events and rewards have also received a festive makeover.

Macabre Horse

A grim steed with a higher satiety limit — the Macabre Horse — has also returned.

It also consumes no extra satiety when returning home without the Exile.

You can purchase the Macabre Horse from wandering merchants. Who knows where they managed to catch it?

Other Changes

The shield effect of Elena the Unwavering is now shown in the active effect panel, where you can see its remaining duration and how much damage it can still absorb.

The Torture Chair tooltip now clarifies that the Forsaken Hermit can be found not only in 3 Skull locations but also in the Forsaken Hermit's Hut.

We’ve rebalanced Oath of Flame to emphasize its mechanic of stacking burn effects. The poleaxe’s base fire damage has been reduced, but the bonus for each burn effect on the Exile has been significantly increased. Now, the weapon will be weaker with only a few burn effects active, but it can deal considerably more damage as those effects stack.

Grim Soul Hub Changes

New Offers

For All Plagued Day, Grim Soul Hub now offers special packs designed to help you clear the Abandoned Leprosarium on Heroic and Legendary difficulties.

A limited-time Sepulchral Direwolf offer is also available, along with a bundle of festive chests that includes the new Flesh Chest.

See you in Lubenia!
